Which check can I have? NHS Health Check Wellbeing MOT
Age eligibility 40-74 18+
Frequency Every 5 years Annually
Already been diagnosed with Cardiovascular Disease or Diabetes Cross Tick

 

If you're not eligible for an NHS Health Check our Advisers will offer you a Wellbeing MOT instead.

 

If we're coming to your workplace for a Wellbeing Visit, your private timeslot will be for a Wellbeing MOT. Please feel free to discuss booking in for a seperate NHS Health Check with your Adviser.

 

What does each health check involve?

To get started either you, or a professional such as a doctor or social prescriber, completes the online referral form.

We'll contact you


A friendly Wellbeing Adviser calls to triage your referral and book you in for either an NHS Health Check or a Wellbeing MOT, depending on your eligibility.

Step 3

Support upwards icon

Private appointment


At your appointment your Adviser will carry out the Health Check and give you personalised advice about small changes you could make to improve your health.

Follow-up icon

Follow-ups


 

If the check flags anything for further investigation you will be signposted to your GP.

We will send you a follow-up survey at 3 and 6 months to see how you're doing.
